Results of Indicator 8A The percentage of toddlers with disabilities exiting Part C with timely transition planning for whom the Lead Agency has developed an IFSP with transition steps and services at least 90 days, and at the discretion of all parties, not more than nine months, prior to the toddler’s third birthday.

Narrative Description of Indicator
Data reported for Indicator 8A is based on a review of all 5,238 Early Intervention Records for children who transitioned from 7/1/22 and 6/30/23. To report the target data for these indicators 8B and 8C, MSDE generated State and local reports throughout the reporting period from the statewide Part C database, and validated data in conjunction with LITPs. The data for Indicator 8B were obtained from monthly reports generated by MSDE and include all children 24-months and older. Reports are uploaded to a secure server for download by both Part C and Part B staff. The reports for Indicator 8C are based on the calculation of the number of days between the date of the transition planning meeting and the child’s third birthday. The number/percent of meetings held within the timelines and the reasons why meetings are not held within timelines are provided. When the date of an untimely transition planning meeting (date later than 90 days before the child’s third birthday) is entered into the database, a prompt appears requesting that the reason for the late meeting be entered. A. During the reporting period, 5,183, or 98.00%, of the records reviewed by MSDE and LITPs had transition steps and services (n = 5,289). B. Between 7/1/22 and 6/30/23, local education agencies were notified of 100% (5,289) of potentially eligible children who transitioned during the time period (n = 5,289). C. Between 7/1/22 and 6/30/23, 98.84% (5,228) of children who transitioned had a transition planning meeting within the timeline or there was a documented family-related reason for the delay (n = 5,289) LITPs that did not attain 100% compliance for subindicators A and/or C, were required to develop and implement improvement plan strategies and, as necessary, received technical assistance from MSDE. 8A - The percentage of toddlers with disabilities exiting Part C with timely transition planning for whom the Lead Agency has developed an IFSP with transition steps and services at least 90 days, and at the discretion of all parties, not more than nine months, prior to the toddler’s third birthday.8B - The percentage of toddlers with disabilities exiting Part C with timely transition planning for whom the Lead Agency has Notified (consistent with any opt-out policy adopted by the State) the SEA and the LEA where the toddler resides at least 90 days prior to the toddler’s third birthday for potentially eligible Part B preschool services (Transition Notification).8C - The percentage of toddlers with disabilities exiting Part C with timely transition planning for whom the Lead Agency has conducted the transition conference held with the approval of the family at least 90 days, and at the discretion of all parties, not more than nine months, prior to the toddler’s third birthday for toddlers potentially eligible for Part B preschool services.
